What to have ready for our call
When our patient coordinator calls you, they will ask for a few details from your medical insurance card and read each one back to you. Having your card in hand makes the call quick.
From your card
- Plan name and insurance company. Usually the largest words on the front.
- Member ID. Read it exactly as printed, letters included.
- Member name. If this is someone else (a spouse, for example), we also need their date of birth.
- Provider-services number on the back. This is the line our office calls, often labeled "Providers" or "Provider services".
If your tooth loss came from an accident or injury
- The date of the injury
- Whether any other insurance is involved, such as auto insurance, workers' compensation, or a liability claim
On the day of your visit
- Your medical insurance card (we scan it at check-in)
- A photo ID
- A list of the medications you take
Your privacy: we will never ask you to text a photo of your insurance card.
Text messages are not a safe place for a member ID. We take the details by phone and enter
them directly into your secure patient record.
